Between 25 November 1915 and 10 Jun 1917 Frederick served with the Battalion in India as part of Karachi Battalion, 4th (Quetta) Division, Indian Army. In April 1917 the Battalion transferred to the Quetta Brigade. He was transferred to 6th Battalion, Royal Defence Corps with Regimental no. 60624 on 27 July 1917. At a medical at the Military Hospital in Gravesend on 9 August 1917 it was noted that he had been severely drenched by monsoons during his service in India and was admitted to hospital with fever and ague. He was in hospital for three months and lost flesh and weight and was unfit for duty of any kind on his discharge. He had been invalided home in March 1917 and joined Gravesend station but had been unfit with occasional attacks of ague. He ended military service on 18 September 1917; His military character was recorded as as Very Good and 'This man is steady and well conducted". He had also been recorded as "Has a debilitated appearance, occasional attacks of ague. Does not seem physically fit for a day's work" and was recommended for discharge as being permanently unfit. As of between 1918 and 29 September 1939, Frederick John Millwood lived at Holmsdale Road (now under Holmsdale House/Stewards Holte Walk) 3, New Southgate, Greater London.
